Bemi Care Human Services LLC is a NJ Division of Developmental Disabilities approved provider of services to persons with cognitive and intellectual challenges, physical limitations and/or learning disabilities. In this role, the agency’s Support Coordinators act as an intermediary to help its clients gain access to services that are available through various state funded programs.
As such, you will find the Support Coordinators:
Evaluating the needs of the client
Identifying the services that are available from the state
Developing a plan so that these services can be obtained
Coordinating the delivery of various supports and services
Advocating, when necessary, for the client to gain more services
Following up with the families and/or advocates of individuals who are receiving support services
