Bemi Care Human Services LLC is a NJ Division of Developmental Disabilities approved provider of services to persons with cognitive and intellectual challenges, physical limitations and/or learning disabilities. In this role, the agency’s Support Coordinators act as an intermediary to help its clients gain access to services that are available through various state funded programs.

As such, you will find the Support Coordinators:

  • Evaluating the needs of the client

  • Identifying the services that are available from the state

  • Developing a plan so that these services can be obtained

  • Coordinating the delivery of various supports and services

  • Advocating, when necessary, for the client to gain more services

  • Following up with the families and/or advocates of individuals who are receiving support services

Community Based Habilitation seeks to develop and/or enhance the skills of persons with cognitive and intellectual challenges, physical limitations and/or learning disabilities to better navigate their world.

Accordingly, services are delivered in a one-on-one setting so that the individual can develop greater confidence and independence. This service starts off within the home of the individual being served and then it extends into the community.

Based on current regulations, the client’s family member can be the provider of the Community Based Habilitation services if he or she meets the qualifications and requirements set forth in the regulations from NJ DDD.

The main focus of Community Inclusion is the provision of opportunities for persons with cognitive and intellectual challenges, physical limitations and/or learning disabilities to become more involved in the communities in which they live.

Accordingly, services are delivered in a small group setting of one to five individuals. This group focus helps the individuals to develop socializing skills that allow the client to better navigate their environment. It also provides peer support.

Respite is a unique service that is provided to the families and/or caregivers of persons with cognitive and intellectual challenges, physical limitations and/or learning disabilities. This service provides the family, advocates and loved ones of the client with a break from care giving when needed.

When arranged, respite services allow caregivers to leave the home for a designated period of time to engage in social and other activities. During that time period the staff from the agency takes care of the client. Respite can be in the following forms:

  • Inside the home

  • Outside the home

  • In a hotel

  • Hours can be daytime, evenings or overnight

  • Weekends are also included

Based on current regulations, the client’s family member can be the provider of the respite services if he or she meets the qualifications and requirements set forth in the regulations from NJ DDD.
